I completed my 12th class with 78% and got 59 in maths in 2012. I am pursuing my higher education.

Yes you can. However, I would suggest you to reconfirm once with the CBSE officials.
The forms will be available in the selected branches of Syndicate Bank. As for the date of commencement, you may visit the respective website for further details.

Yes you are eligible for your improvement exam in 2014. If you can write the same in Sep 2013 which help you avoid lose one year.
