No, you do not have to submit any mark sheet anywhere. In case they ask you for the verification then you can show your latest mark sheet that you have.

Your chances are pretty good this year if we compare it to the cutoff of last year. But this year, as there was only 1 CMAT, the cutoffs are going high. So, if your AIR is around 4500 and Percentile is around 94-95 (General Category) then you can be pretty much assured, otherwise the chances are 70%.
Yes, it is necessary to submit the Final Year result in the ACPC process only. If the result comes after you have submitted the Form through ACPC, then you'll need to personally go to ACPC office (In LD college, Ahmedabad) and submit the results before the last date of for filling.
