Are the admission forms out of Manipal Institute of Technology?

Yes, MIT Manipal has released the application form for 2018 admissions.
Go to this link to fill it up: https://apply.manipal.edu/content/mu/admission/indian-students/how-to-apply/apply-step-1.html.
A fee of Rs 2000 will be charged which contains Rs 600 as application fee and Rs 1400 as exam fee.
You can also give re-exam of the entrance once you are not satisfied by the first one. Best of them will be taken as your score in MUOET. For re-exam a nominal fee of Rs 1000 will be charged.

The applications for MIT, Manipal 2018 are out on the https://manipal.edu/mu.html. The applications will be open till January. In May you'll be prompted to select a date for your Manipal University online entrance test which happens in the month of April and May. Post this, counseling will take place in June at various colleges under Manipal University across India. All the best.
