The term nutritionalist is not protected in most countries, allowing most anyone to use the title. Thus, you could not call all nutritionalists doctors. Dieticians on the other hand a accredited and doctors.

The health issues most of the time depends on our diet and nutrition is also a branch of medical science, which deals with these problems. So, someone who is a nutrition expert is a doctor.

"Doctor" is a level of educational attainment, although it is often used to refer to medical professionals.
