It depends on the university whose entrance test you are appearing for. If the criteria for admission is only cracking entrance exam then there is no chance of entry.
Otherwise, admission will be based on the percentile of the latest degree achieved by you. -

Yes, it is mandatory to qualify LPUNEST for Ph.D. Forms are already available on the website i.e http://www.lpu.in/. Last date to apply for LPUNEST is 21st December 2017. The exam will be held on 24th December 2017.
