Can I opt for medical stream after class 12th?

My chemistry and maths are not strong.

Yes, if you are interested in medical stream then it is necessary to be perfect in biology.

If your question is for MBBS or BDS, then it is not possible because bio is a compulsory subject and it is one of the criteria for the selection.

It is not possible Arpita. Biology subject is compulsory in 12th for giving any medical exam further to pursue MBBS and the only way you to repeat 12th is if your board have that provision.
