My brother has completed 11th & 12th with Karnataka board, he didn't take biology as an option. Now he regrets his decision. He wants to study medicine or paramedical field. Can he study again with the same board?

Yes, you can study again. There is no problem in that. But, think before doing this as this will waste your 2 more years. But, if you are very much interested in studying again, then it is good, as you would have already made decisions for future scope. All the best.
