Do I need to enclose any documents along with my Berhampur University admission form?

<p>Selected candidates have to present a set of documents for verification purposes. The aspirants selected through CPET for PG course admissions need to submit the following documents (original):</p><ul><li><span lang="EN-GB">Print copy of duly signed common CPET application form</span></li><li><span lang="EN-GB">Print copy of SAME online fee deposit receipt</span></li><li><span lang="EN-GB">Class 10 and Class 12 certificate</span></li><li><span lang="EN-GB">Bachelor's mark sheets</span></li><li><span lang="EN-GB">Downloaded copy of transfer certificate/college leaving certificate in original</span></li><li><span lang="EN-GB">Registration receipt (only Berhampur University candidates)</span></li><li><span lang="EN-GB">Migration certificate in case of students not belonging to Berhampur University (can be submitted within three months)</span></li><li><span lang="EN-GB">Certificate from the appropriate authority for availing of reservations/exemption, if any</span></li><li><span lang="EN-GB">Provisional/original certificate of the last degree obtained (if the last degree examination passed is other than the qualifying examination)</span></li><li><span lang="EN-GB">Conduct certificate from the last attended institution head</span></li><li><span lang="EN-GB">Five copies of your recent passport-size coloured photograph</span></li><li><span lang="EN-GB">Certificate from a recognised medical practitioner indicating the blood group of the applicant</span></li><li><span lang="EN-GB">Application for accommodation in the university hostel, if required</span></li></ul>
