How can I clear my concepts of physics and chemistry? Which textbook should I refer?

    You need a lot of self-study and a good guide who can guide you to clear your concepts.
    Consult your teachers or your parents try to focus on both subjects and try to solve more and more questions.

    If you do not skip even the minor details also, you can develop a better concept. Most problems arise, just because we are mindful of important topics only. Be exhaustive in your study.

    For chemistry ncert is sufficient, but if you need to refer some other books then go for ABC. For physics I suggest you SL ARORA.

    Best Text books for Physics & Chemistry are very well known. For Physics HC Verma is good. You can check Irodov, but it is way too difficult to understand. For Chemistry, you can opt for Ebing or NCERT Book.
