How to apply for a duplicate marks cards and wat is the fee for 2 marks cards - bangalore university.

I have not received the marks card from the my college but when i enquirer about the same they told they dont have it with them, so they suggested me to log a complain in the near by police station and also a affidavit and submit it to the university. wat should be the matter for the affidavit?
