I am a little weak in physics and ok in chemistry and good in bio which stream should i choose

With some external and web assistance in Physics and dedicated work you will be able to excel in this area also. So for your +2, you can opt for Science stream if you aspire medical profession.

Physics is Mathematical and it may not be suitable for you, in view of your particulars. You'd better pursue Biology combination.
