I appeared for boards in 2016, gave improvement exam in 2017 and appeared for JEE (M) in 2016 & 2017. Am I eligible for JEE (M) 2019?

No, you are not eligible for JEE Mains in 2019. It is stated in the guidelines of JEE Mains the maximum attempts are 3 in consecutive years.
