You need to clear GRE and TOFEL (English Proficiency Test). A score 315-320 is good to get calls from top universities of US.

For country or university you can take assistance from a good advisor or any coaching institute offering preparation of GRE exams. Generally, students opt for US as it has good education level and many good universities, apart from UK, NZ, Australia and Canada.
