I passed 12th class in science with 83% and want to study MBBS. I got 6569 rank in West Bengal medical entrance. Which course should I opt?

If you want to study in West Bengal, you have to prepare well & try for better institutes next year because you must score a rank under 2000 if you want a good medical college there since the private medical institutes in West Bengal are not good for studies.
