If I want to apply to US universities for the fall admissions, when should I have given my GRE and TOEFL?

I have not yet taken both the exams. At the most, I will be able to write them by February 2012. Would this be too late for applying for the fall 2012 intake? Also, if I give my GRE twice, which score would be considered? The first attempt's score, the highest or the average?
