According to the previous year's JEE counseling guidelines, you are required to submit a medical Certificate during JoSAA (JEE) counseling. The format of the required medical Certificate is available in the JoSAA guideline and notification pdf. You can visit the nearest government hospital to get your checkup done and fill up the form accordingly. This includes height, weight, vision, etc. You need to get this form signed and get the seal of the designated authority. During the counseling process, you need to submit this format along with other documents. -
