Is it mandatory to have Masters to pursue Ph.D in USA? Can I directly apply for Ph.D with Bachelors?

In USA, you have a vast option of doing Ph.D. . You can pursue after it after you Bachelors also. You may couple the Masters and Ph.D. . programme, or first obtain a Masters degree, and then apply to a Ph.D. . programme.
