Is it necessary to fill out the OEAF? Or is there any provision for a physical examination application for NIELIT?

Yes, it is compulsory to fill out the Online Examination Application Form (OEAF). There is no provision for offline or physical forms. Here to note is that the Physical Examination Application Form has been discontinued from the July 2013 Examination onwards.
