Yes, it is possible to pursue MBBS in USA/Caribbean on the basis of your NIOS. NIOS is a valid government board. You can pursue MBBS directly. Course duration for MBBS/MD in USA will be 5.5 years.
You can get direct admission in MBBS/MD programmes in USA/Caribbean and UK after your class 12th. You will be eligible for MCI Screening also in India. Package starts 35 Lac for the complete course including fooding and accommodation.

How did you pursue your 12th without completing 11th?Also, there is no direct MBBS in USA or Canada. You must complete your Bachelor's degree and take an exam called Medical College Admission Test to study in a medical school. MBBS programs are very competitive for international students and possibility is less, you must write to the universities yourself and check if NIOS is accepted by the particular universities for MBBS admission.
