Yes, it has extended to 28th May and self-attested is acceptable but carry original with you for cross verification.

Yes, the dates have been extended till March end. The documents can be self attested.

No. You have to take a print out of the form available on our site i.e. www.siescoms.edu and send it via post after filling it up along with the necessary documents.
