On downloading the PDF form I could see few other sections with questions for Retail,E Business etc Do I Have to fill up those ?

If you are applying for those courses, you need to fill. If not, no need to fill.

Yes it is to be filled and submitted.
When your profile will be evaluated at the time of further processes it would be considered.
It will also be discussed during your personal interview.
