On what basis is the cut-off percentage calculated for Mithibai College: on overall 12th percentage or some top-3 or 4 subject's percentage?

The cutoff percentage are calculated on the basis of overall subjects in HSC. As you said, you have 89%, then you have a chance to get admission in Mithibai College.
