Procedure to get pending certificate from south Gujarat university for 1999 batch

I did my degree in 1999 from South Gujarat University, however certificate was not issued that time. Kindly advise how to get my pending Certificate for the same?, Is there any procedure to submit online request?. Is there any particular form for this ? Where to submit this form ? Thanks
