No, a gap certificate isn't required to fill out the NEET application form itself. The requirement for a gap certificate comes during the counselling stage, that too, if you have taken a break in your studies after passing class 12.
You can proceed with filling out the NEET application form without the gap certificate. However, it's a good idea to be prepared and get the certificate in advance in case it's needed during counselling.
