Your XAT score is not good and will not be able to get you admission. However, your good academic record and good MAT score can get you admission in top 25 B-schools under MAT as they accept admission application from candidates having score above 75 percentile. I suggest following steps:-
1. Google "Top B-schools under MAT"
2. Go through the list and short list 3-4 colleges from this list which are closer to your home town/state. Go to their website and check their placement assistance program, fees infrastructure and admission procedure. In most cases you will be able to download their application. Apply and wit for their response.
