The admission procedure differs from one institute to another and one course to another. As for fee structure, it would be tough to quote a specific number as it keeps on changing almost every year and differs from one course to another. So, I would suggest you to contact the shortlisted institute for specific admission details.

To become a Game Designer, you must get a good knowledge of 3D software as well as preproduction. Usually in Games the environment is created in 3DSMax and Character animations in Maya, Only the models are made with less polygons and more textures. There isn't any medical entrance examination that I know of that is yet to be conducted for the coming academic year. You may wait for one year and appear next year.
