U can pursue MBA from many good institutes depending upon which u exam u clear and u get a final call letter.
U can appear for
CAT- includes all IIMs, SPJain, Welingker, IMT, MDI etc.
MAT-includes Alliance, Christ, IILM, Amity ,IMS, Apeejay etc.
JMET-includes all IITs etc.
SNAP- includes all the symbiosis group.
and many good options like IIFT and FMS etc.

The long gap in your BCA and work experience is not the concern. The concern is your 3 years undergraduate degree. You have heard right that USA requires 4 years of undergrad education. There are a few universities that accept a 15 year degree but those universities would not be very well known globally and it would not be worth spending so much money. I would recommend you to work for another year, pursue a distance learning post graduate diploma side by side from the UGC recognized university and apply next year.

actually the reality is that iipm is not recognised by ministry of hrd &it does'nt comes under the ugc regulations.it's true that iipm will give the degree to u. if it 'll remain unrecognised then it'll cause a lot of problems.

Hi Vitthal,
Most of the German Universities ask the GRE score from Indian students especially for the subjects like- mechanical, aerospace, electrical,automobile engineering etc. SO its always advisable to give GRE exam before applying also if your desired course is taught in english then TOEFL is also required. Kindly check with the concern university for more details.
