Yes, to get an admission in Tolani Maritime Institute (TMI), you have to appear in both IMUCET and TMISAT.
IMUCET helps you get through all the government IMU campuses on the other hand TMI, which is a private institute conducts it's own separate Computer Based Test which majorly contains questions from Maths and Physics.

SAT is not designed for medical school. You will be eligible to apply for an UG degree after SAT. After completing your Bachelors you must clear the MCAT before seeking admission in a medical school. This basically applies for US and Canada. Different countries have different. Policies.

With 3426 rank, it's not possible to get admission in any of the NITs as there are only around 559 seats in all NITs. For a general category student to get a seat in any of the NITs, his/her AIR should be less than 1000.

Yes, it is possible but you have to wait till the last rounds of admissions. Generally students with higher ranks get VIPS during the first and second round of counselling but sometimes a student leaves the seat after freezing it because they get admissions in their desire colleges which allowed the students with lower ranks to avail that seat during third round or even in sliding or spot round.
