There is nothing like a format as far as my knowledge goes.They are quite common and are issued by the Principal of the Institution.The letter must express a previous knowing of the candidate and the personal suitability for further study or job.The letter is titled TO WHOMSOEVER IT MAY CONCERN

The format of letter of recommendation is normally available from the University which you intend to pursue MS or from their agents. Most of the universities in US requires 3 LORs, and these LORs have to be academic or professional, i.e . if you are a working professional you need to have at least 1 Professional LOR and the remaining 2 LORs can be academic i.e. form your professors or lecturers.
