Should I refer any other book except NCERT for physics and chemistry in class 12th so as to score up to 95%?

Is there a need to refer any other books for extra information and knowledge because for question practice I have previous years' question papers?

For physics NCERT is not a good choice. As the language used for explanation is very typical for some topics so for 12th one needs a guide book I would suggest you to use SL Arora its a nice book with ample of questions subjective as well as objective.
