After a candidate has received the provisional allotment letter online, they are supposed to physically report at SRM Medical College with the following documents, (along with two sets of photocopies of the same):
· Provisional allotment letter
· NEET admit card, score card & rank card issued by NTA
· Mark sheets of Class 10 and 11

- Admit card
- Scorecard or rank letter
- Provisional allotment letter
- Original Class 10 certificate
- Original Class 12 mark sheet
- Caste certificate (SC/ ST/ OBC category candidates)
- PwD certificate (if applicable)
- ID proof (Aadhaar/ PAN card/ Driving License/ Passport)
- 7-8 passport-size photographs
