- Service agreement with concerned body/church
- 2 Colour passport photographs of Parents and Guardians
- Undertaking that all certificates are original
- Baptism certificate
- Letter from the Pastor in-charge of the church regarding Church Membership with dates
- Letter of Sponsorship from Body/Church of the CMC Ludhiana Society
- Domicile certificate
- Admission Written Test (AWT 2025) scorecard
