Certificate (Diploma in Coaching) 5. Conduct certificate issued by the institution last attended 6. Caste Certificate (SC, ST, SEBC) issued by the competent authority 7. Medical Certificate in original available in application form Govt. registered Medical Officer 8. Residential Certificate (issued in Form No.III by Tahasildar) 9. Original money receipt (with Form No.) 10. Three nos. of attested recent pass port size photograph 11. Certificate to the effect that he/she has passed M.E. standard in odia language. 12.
