What are the requirements for me to pursue my pending PGDBA+MBA course?

I had taken admission in PGDBA + MBA in 2013-14 & the course grace period is till 2016 but due to some personal and work problems, I cannot complete my course. Now, I want to continue the same but I don't have my enrollment no. with me. I would request you to check your records if it is possible.

Your good CAT score, and your graduation marks must be above 50 % if you are from open caste, and 45 % from backward class.
