1. Baptism certificate
2. Letter from the Pastor in-charge of the church regarding Church Membership
3. Letter of Sponsorship from Body/Church of the CMC Ludhiana Society (for B.Sc. Nursing Course only)
4. Proof of Parents' name
5. Self Undertaking of Gap in study after 10+2 (if applicable)
6. Service agreement with concerned body/church
7. 2 Colour passport photographs of Parents and Guardians
8. Undertaking that all certificates are original
9. Domicile certificate (if available)
10. Affidavit for College and Hostel Rules.
