What documents are required for JEE Mains under the PwD reservation? Is the PwD certificate format provided by NTA the only acceptable one?

I have a PWD certificate provided by the government with a UDID card.

Typical requirements for JEE Main under the PwD reservation are:

1. Certificate of PwD as per the format specified by NTA (National Testing Agency). This certificate is to be issued by any hospital or any other recognised medical authority and it shall indicate the disability along with its percentage.

2. UDID Card (if available) : Government-issued Universal Disability ID card.

3. Include: Photo ID / include a government-issued photo ID for verification

In the case of a PwD certificate with UDID card but it is not issued in NTA format, candidates are advised to get this confirmed and if have been reissued must be converted into NTa format
