You need to submit your 12th mark-sheet scan to university. Get the admission letter and invitation letter from university and then apply for visa.
Ensure you take MCI clearance before you leave country.

You can get direct admission in MBBS/MD programmes in USA/Caribbean and UK after your class 12th. You will be eligible for MCI Screening also in India. Package starts 35 Lacs for the course including fooding and accommodation. -
Before taking admission there, you need to check if it is approved by MCI or not. As you will not be able to work in India otherwise.
