Your focus during the academic years must be on understanding the basics and making notes. When you start with your internship, you can begin focusing solely on NEET PG preparation. Start your preparation by attempting the question papers of previous years.

According to my knowledge, MBBS 2nd year or 4th semester is the best time, to start preparation for NEET PG. Preparation tips:
    1- know the syllabus.
2- Know the paper pattern and marking scheme
    Exam pattern of NEET PG:

3- Try to solve the previous year's question papers.

Try to catch up concepts from the 1st year only. Then it will be very easy to correlate things in the later years and after 3rd year you can start solving Exam based sample papers.
