What to do if there is a discrepancy in the name on the certificate or JAM scorecard in CCMN document verification process?

At the time of registration applicants should write their name as filled in JAM application form. However, during document verification process candidates can give an affidavit on INR 50/- Stamp Paper stating that the two names belong to the same person and if found false at any stage then his/her seat in CCMN counselling will be cancelled.
