What was the minimum percentile alloted to get nitk in cat2014? Also how to download application form?

As far as cut-off percentile is concerned for NITK, its not yet been finalized.
From this year both CAT and GMAT scores will be considered.
Application form will be available on NITK website : www.nitk.ac.in
Application forms will be available after Jan 2015.
