    Self declaration form is attached with your admit card. As you download it from the official website of NTA, it consists of more than one page and the last page is a self declaration form.

    • Download the Admit Card
      Search and download admit card. The self – declaration form is also annexed to this document.
    • Print the Admit Card
      Take a print out of the whole admit card along with the self-declaration form.
    • Complete the Self Declaration Form
      Fill the necessary fields that would summarize about the client's health state or travel history.

      Indicate any response on the printed space, take the form to the examination center as dir

No. NTA has clearly mentioned that ot will not change the dates of JEE Main exam of there is a clash of dates with any other examinations.
