Where can I find the self-declaration on NTA site?

The self – declaration form is also annexed to this document.</li><li>Print the Admit Card<br>Take a print out of the whole admit card along with the self-declaration form.</li><li>Complete the Self Declaration Form<br>Fill the necessary fields that would summarize about the client's health state or travel history.</li><li>Carry the Form<br>Indicate any response on the printed space, take the form to the examination center as directed.</li></ul>
