Which engineering colleges Accept JEE MAIN score but have Individual forms to be filled Before the exam and When do they become available?

IITs NITs do not need form filling before the entrance exams to be taken. But several colleges such as IIIT-Hyderabad release their own application form but accept JEE MAIN score. I want to know the list of all such colleges, when to fill these forms and how many forms are advised to be filled?
