Which name should we write in medical Certificate, from 10th Certificate or the admit card issued?

Both name should be the same. The name and date of birth written on 10th Certificate are authentic and valid for all admissions, exams and for employment purpose. If different name printed on admit card can create problems. Write the correct name as written on 1th Certificate.
