The National Testing Agency (NTA) does provide a declaration form in lieu of the General-EWS certificate Annexure IB:

Declaration by the Candidate in Lieu of GEN-EWS Certificate
This is a signed declaration form that candidates can upload online in lieu of a General-EWS certificate.
