Pattern of Entrance test:
The Entrance Test will be of 3 hours duration consisting of 200 multiple choice questions divided as follows:
Mathematics:80 questions
Computer Awareness: 60 questions
Analytical Ability, Communication Skills & General Knowledge: 60 Questions

PLANNING can be technical planning like town planning, civil planning etc. This will need architecture or civil engineering as base knowledge. (Another planning can be non tech planning like planning a wedding or management or family. I guess this planning is not the thing, you have wanted for discussion). Architecture is science & engineering which teaches drawing, engineering materials, strength, needs, engineering calculations etc which will be required to design any structure like building, bridge, road etc. Planning starts hereafter. It teaches how to implement architecture knowledge into broder aspect like plan a town, plan a city, plan a museum, plan road network etc.

Hi Sahil, there are lots of countries that are good. But what matters is which subject you are going for. You have posted the question under IT so let me tell the country best for this subject is India. And if you insist on studying abroad then you can opt for UK, USA, Aus, Canada, Singapore.

TOEFL is a test of English and can be easily prepared at home. If you would like coaching then you can find centres from the same link above.
Not all countries require GRE scores. But TOEFL or IELTS is mandatory for Indian students except in Singapore.
Documents required to apply for admission- Class 10 and 12 marksheets and Passing certificate, sem wise or year wise marksheets, degree certificate or provsional certificate, Statement of Purpose, Letters of Recommendation(2), Work experience letters from all employers (if any), CV (if you have work ex or are applying for MBA), A copy of your passport(1st and Last Page), IELTS (6 min for UG courses and 6.5 min for PG courses). IELTS can be submitted later on if not already written.
