Top universities are MIT, Stanford, Berkeley, California, Hawai.
Fees is in the range of $25,000-35,000 per year for two year course. Scholarships are available on merit based and only after admission has been taken and first instalment has been paid. Approximately, $600 per month is the living cost including transportation, housing & food. $250-500 is additionally required for insurance and books.
GRE and TOEFL/IELTS scores are mandatory. GPA OF 3/4 IS THE MINIMUM REQUIREMENT IN DEGREE.
All marksheets from 10th onward, GRE & TOEFL score, two letters of recommendations, preferentially one from school and one from your college is accepted.

Good to hear that, you can do Certified Management Accountant [CMA] from Institute of Management Accountant [IMA - USA]. This is equal to ICWAI of India, once you complete the CMA in USA, you can get the Membership of ICWAI without writing any exams in ICWAI.
