Doing both at the same time is manageable to some extent but prioritizing both of them is quite difficult. I would suggest that if you are planning to do any professional course like CA CS CMA or any other, it would be best if you pursue your graduation from distance learning. If you take regular college it would get really difficult for you to manage both of them (saying that because of my personal experience)

To pursue B.Com and CA together you can do B.Com from correspondence and do your CA
the syllabus for B.Com is same as CA
The eligibility criteria for CA final is you must be graduated from a certified university so its better to pursue B.Com with CA.

In the Indian context, BHMS is better as it will give you a certified doctor status which is not there in the BPT. You will have a better career in the BHMS than in BPT in India. From BHMS you could also transition to allopathy by doing a few courses. See there are no people with no dental problems especially in the pediatric age group or geriatric age group they have some or the other problem with their teeth. Also as unless people underrated BDS most of them won't choose to go through BDS so it should be good for your future if you have a good passion for the Dr you are becoming.
