Getting a 2011 session admission form would depend on the record keeping policy of the institution. Most colleges or universities in India keep records for a period of 5 to 10 years,  

 the probability of getting hold of the original form or a hard copy will be very low, unless a copy is there in the archives or it has been specifically preserved. For a mark sheet from that session, you can contact the exam section of the university since you have proof of admission (roll number, ID, etc.). If lost, most of the time the duplicate is issued after records verification along with some fees and an affidavit.

Students with the eligible criteria and all the necessary documents can apply for the admission. The university will also assist the applicants with the further process and documentation to their applicants.
