Prepare for the SAT and TOEFL and apply for Bachelor's degree courses. Since you have already completed your polytechnic course a BS degree will be beneficial. Please visit university websites and check the programs they offer, see what interests you and check the entry requirements. Prepare accordingly.

Studying medicine is really difficult in US for international students as the universities give preference to American citizens for medical courses. So, international students getting admitted to an MD course are very rare.

The standard route to this is not as such reading any specific books at this stage (I'm not discouraging you from reading popular books on cosmology, which you should whenever you can). The eligibility for joining merchant navy is:
1. Passed class 12 with physics, chemistry and mathematics.
2. Candidates have to be unmarried and Indian citizens (male or female).
3. Normal vision is required but glasses up to plus or minus 2.5 may be allowed.
