Most of the institute give admission on merit-based and most of them also conduct the entrance exam. It all depend on institute, so for entrance you have to fill the registration form and then the university allots you the college in merit-based admission. You also have to register yourself at the university web site and then according to your marks in boards university allots you the college.

Of course, it is a safe place. I do not think you need to worry about it. City is also safe. Other thing is that IIT Patna comes under the center, so there is no chance of ragging or anything. IIT Patna is in Bihar and that area is less populated.
